The American College of Obstetricians and Gynecologists (ACOG) recommends that girls have their first gynecologic visit between ages 13 and 15. But what does that visit involve—and what are some signs it may need to happen even earlier?

What to Expect at the First Visit

A first gynecological appointment is more about building trust than performing exams. In most cases, a pelvic exam is not needed unless there are specific health concerns. Instead, the visit focuses on:

  • Asking questions about puberty and menstruation
  • Learning about healthy habits and hygiene
  • Discussing the HPV vaccine
  • Creating a safe space to talk about body changes, sexuality, and emotional health

This appointment is a chance to meet a provider who can become a trusted resource as the patient matures. We want young people to feel heard and supported, and to know they always have someone they can turn to with questions or concerns.

Reasons to See a Gynecologist Sooner

While 13–15 is the general guideline, there are situations when it’s best to schedule a visit earlier. These include:

  • No period by age 15 or within three years of breast development
  • Very heavy, painful, or irregular periods
  • Signs of hormone imbalance, such as excessive hair growth or acne
  • Recurrent urinary or vaginal infections
  • Ongoing pelvic or lower abdominal pain
  • Concerns about sexual activity, contraception, or sexually transmitted infections (STIs)

It’s also important to seek care if a young person is experiencing emotional distress related to puberty or body image. Gynecological visits aren’t just physical—they also support mental and emotional wellness.

HPV Vaccine and Preventive Health

The first visit is a great time to talk about the HPV vaccine, which is recommended for preteens and teens. This vaccine protects against the most common strains of the human papillomavirus that can lead to cervical cancer later in life. Preventive care now can reduce risks in the future.
