In vitro fertilization has been helping people build families since 1978. In those nearly five decades, the science has advanced enormously and 2026 is no exception. From improvements in embryo assessment to expanding genetic screening capabilities to a sharper understanding of the hormonal protocols that optimize outcomes, IVF today looks meaningfully different from IVF even five years ago.

If you’re considering IVF, just starting the process, or trying to understand whether it might be right for you, here’s a current look at what IVF involves, what’s changed, and what to realistically expect.

What IVF Involves: A Quick Recap

IVF is a multi-step assisted reproductive technology process. In simplified terms:

  • Ovarian stimulation: fertility medications stimulate the ovaries to produce multiple eggs rather than the single egg released in a natural cycle
  • Egg retrieval: a minimally invasive procedure, done under sedation, to collect mature eggs from the ovaries
  • Fertilization: eggs are fertilized with sperm in a laboratory setting
  • Embryo culture: fertilized eggs develop into embryos over five to six days, typically to the blastocyst stage
  • Transfer: a selected embryo is transferred into the uterus; any remaining viable embryos can be frozen for future use

 

The path to IVF varies by patient. Some women pursue it after trying to conceive naturally without success; others go directly to IVF based on a known diagnosis (blocked tubes, severe male factor infertility, premature ovarian insufficiency, etc.). The evaluation process — labs, imaging, a semen analysis for male partners — helps determine whether IVF is the right next step and how to optimize the protocol.

What’s New in 2026

AI-assisted embryo selection. Artificial intelligence tools are now widely used by embryology labs to assist with embryo grading and selection. These systems analyze embryo development patterns and predict viability with greater consistency than visual grading alone, helping identify the embryo most likely to result in a successful pregnancy.

Expanded preimplantation genetic testing (PGT). Preimplantation genetic testing for aneuploidies (PGT-A) — which screens embryos for chromosomal abnormalities before transfer — has become more routine. This allows for the transfer of chromosomally normal embryos, reducing miscarriage risk and improving success rates, particularly for women over 35 and those who have experienced recurrent pregnancy loss.

Newer additions include PGT for polygenic conditions, which can screen for complex traits with a genetic component. The ethical and practical landscape here continues to evolve, and it’s an area worth discussing with your care team.

Freeze-all and frozen embryo transfer (FET) cycles. The trend toward freezing all embryos and doing a frozen transfer in a subsequent cycle — rather than a fresh transfer — has continued to grow. Frozen transfer cycles allow the uterus to recover from stimulation before implantation, and evidence suggests outcomes for frozen transfers now match or exceed fresh transfers in many patient populations.

Mitochondrial and time-lapse imaging. Time-lapse incubators allow continuous, undisturbed monitoring of embryo development — giving embryologists more data points without removing embryos from the controlled environment. This technology is increasingly standard in high-volume IVF labs.

Ovarian tissue cryopreservation. For patients facing treatments that may compromise fertility (such as certain cancer therapies), ovarian tissue freezing offers an expanding option to preserve fertility potential — a field that has seen important advances in recent years.

What to Realistically Expect

IVF success rates have improved significantly, but they remain highly variable and depend on factors including age, diagnosis, embryo quality, and clinic protocols. According to CDC data, live birth rates per egg retrieval for women under 35 using their own eggs generally exceed 50% per cycle at high-performing centers — but rates decline with age and vary with individual circumstances.

What hasn’t changed is that IVF is physically, emotionally, and financially demanding. The medication protocols are intensive. The two-week wait between transfer and pregnancy test is one of the most psychologically challenging parts of the process. The financial burden, for many patients, is significant — IVF is not universally covered by insurance, though coverage is expanding in some states.

Going in with realistic expectations, strong support systems, and clear communication with your care team makes a real difference in how you navigate the process.

Is IVF the Right Path for You?

IVF is one tool in the fertility treatment landscape — and not always the first one. Depending on your diagnosis and history, other interventions (timed intercourse with monitoring, ovulation induction, intrauterine insemination) may be appropriate first steps. A thorough evaluation is the starting point for any fertility journey.
