While HIV/AIDS awareness campaigns have historically centered on certain communities, the reality is that HIV affects people of all genders, orientations, ages, and backgrounds. For women specifically, understanding the facts about HIV testing and prevention is an essential part of comprehensive reproductive healthcare.

HIV in Women: The Numbers Tell an Important Story

Women account for a significant portion of new HIV diagnoses in the United States each year. Heterosexual contact is the most common mode of HIV transmission among women, though injection drug use also plays a role. Black and Hispanic women are disproportionately affected by HIV, reflecting broader systemic inequities in healthcare access and social determinants of health.

Despite these numbers, women are often less likely than men to be tested regularly for HIV, partly because HIV is still frequently perceived as a “man’s disease.” This misconception can lead to delayed diagnoses, which affects both individual outcomes and the ability to prevent transmission to others, including during pregnancy.

Why HIV Testing Is Part of Routine OB/GYN Care

The Centers for Disease Control and Prevention (CDC) recommends that everyone between the ages of 13 and 64 be tested for HIV at least once as part of routine healthcare. For sexually active individuals with multiple partners, annual testing is recommended. HIV testing is also a standard part of prenatal care—all pregnant individuals should be tested early in pregnancy, and again in the third trimester if they are at higher risk.

A positive HIV diagnosis is no longer the crisis it once was. Thanks to modern antiretroviral therapy (ART), people living with HIV can expect to live long, healthy lives. When HIV is detected early and treated consistently, the viral load can become so low it is undetectable, which also means the virus cannot be transmitted to sexual partners, a concept known as Undetectable = Untransmittable, or U=U.

For pregnant individuals living with HIV, consistent treatment dramatically reduces the risk of transmitting the virus to a baby during pregnancy, labor, or breastfeeding. Early diagnosis and proper management make a profound difference in outcomes for both mother and child.

PrEP: A Powerful Prevention Tool for Women

Pre-exposure prophylaxis, or PrEP, is a daily medication that can reduce the risk of contracting HIV by up to 99% when taken consistently. Despite its effectiveness, PrEP remains underutilized among women. If you are at higher risk for HIV—due to a partner’s status, inconsistent condom use, or other factors—PrEP may be worth discussing with your provider.

HIV stigma remains one of the biggest barriers to testing and treatment.
